Begins with the End in Mind

High school students often have difficulty imagining their life after graduation. To help students with Individual Education Plans (IEPs) prepare for life after graduation, they have Transition Plans. Transition Planning is a process that is introduced and reviewed annually during a student's IEP meeting. Transition plans help students explore, discover, and plan for their journey after high school. While in high school, a student's educational team includes the student, parents, family members, case managers, school psychologists, social workers, school counselors, special education administrator, speech and occupational therapists and, the transition specialist, all have a unique role in helping students prepare for tomorrow.

Some of the supports Central's Transition Specialist can provide are:

  • Educating and supporting the transition of a student from special education to post-secondary education disability services.

  • Providing training and guidance on post-secondary admission processes (SAT scores, grade point average (GPA) requirements, financial aid, etc.). 

  • Connecting students and families with community resources to support the student’s post-secondary vocational goal, post-secondary education plans and independent living resources. 

  • Assisting with the pre-employment process, completing job applications, resumes and interviewing tips and tricks.

  • Working with local employers and providing local job leads. 

  • And much, much more!
